Lake Braddock High School Athletic & Activity Registration
*** Grades 9 - 12 ONLY ***
Parents will create one account per family. Students in the same family will need to be registered within the same account.
Concussion Education is now a required step within registration. It must be reviewed in its entirety and electronically signed by both the parent and student. If a paper version has already been submitted for 2020-21, you will still need to complete the Concussion Education step to move forward. This will be our only accepted form of Concussion Education completion moving forward. Signed paper versions will no longer be collected by the Athletic Training Program or Activities Office.

Emergency Care Form information is now collected via the registration process. Please make sure this information is complete and accurate, as it will be shared with EMS in the event of an emergency. Hard copies will only be required for Green or Yellow Day activities.
VHSL Physical Forms will continue to be collected by the Athletic Training Staff or Activities Office. Physicals can either be emailed to Kristyn Wharton at firstname.lastname@example.org (PDF format preferred), mailed to the school, dropped off in the main office, or handed in during our last designated in-person collection (Nov. 20th, 3:30-5:30 pm, Door 14). If you have already submitted a physical for this school year, it is on file and will remain valid until June 30th, 2021. Blank VHSL forms can be found here.
Parent/Guardian Health Screening Commitment Form is an agreement to complete a daily health screening prior to sending your student-athlete to school. We are asking all parents to complete this form electronically. This will allow your child to enter the building for workouts and/or practices amidst COVID-19 procedures. Tryout Card Distribution will be shared via News You Choose, the LB Athletics website, and social media prior to the start of each season. We typically distribute Tryout Cards the week before each season starts. Please keep an eye out for this information as your season approaches!
