  • Pictures will NO LONGER be accepted for valid submission. Pictures are typically dark or are poor quality by the time they travel through the FCPS email server, making it difficult to read them clearly. Please DO NOT send pictures of this document.

PROOF OF VACCINATION REQUIREMENT FOR ATHLETES:

  • Proof will be accepted in one of two ways:
    • Scan an e-copy of the vaccination card & email to LBSSActivities@fcps.edu.
    • Bring in hard copy of vaccination card to a member of the Student Activities Office (E110) where a photocopy will be made and kept on file.
  • Medical ExemptionsStudents who are not receiving the COVID-19 vaccine due to documented health or medical reasons will be exempt from the requirement, but they will need to provide information to their schools. Student-athletes can submit documentation of a medical exemption from their physician directly to their Student Activities Office. Students can do this through a letter from their doctor (on the doctor’s office letterhead) stating the student has a medical condition that prevents them from receiving the COVID-19 vaccine.
  • Religious ExemptionsStudents who are not receiving the COVID-19 vaccine due to religious reasons will be exempt from the requirement, but they will need to provide information to their schools. Student-athletes can submit the Commonwealth of Virginia Certificate of Religious Exemption Form to their Student Activities Office. The form must be notarized.
  • Age ExemptionsFCPS will not mandate the vaccine for student-athletes who are 12-15 years of age. However, vaccination is strongly recommended and FCPS will collect the vaccination information for those 12-15 years of age who wish to provide it.
  • Students who are not receiving the COVID-19 vaccine due to medical/religious reasons or who fall outside the age requirement (12-15 years of age) will be required to submit a negative COVID-19 test weekly.
  • Students who are 16 years of age or older (age as of November 8 for Winter Season) who are unvaccinated and do not have either a medical or religious exemption will not be able to participatte in team activities.
  • In addition, unvaccinated students who are 12-15 years of age who do not provide a weekly negative COVID-19 test will not be able to participate in team activities.
  • Deadlines:
    • Proof of vaccination will need to be submitted by November 8th for Winter Sport Athletes and for anyone participating in Out-of-Season activities (Green/Yellow Days). Winter Athletes will not receive a Tryout Card if this requirement is not met.
    • Exemptions will need to be submitted by November 1st, so that a negative COVID-19 test can be submitted prior to the start of tryouts. Winter Athletes will not receive a Tryout Card if this requirement is not met.
  • For more information, please review the FCPS Athletics Vaccination Requirement – FAQs page.
